How to Connect Patrons to Social Services Provided by DPHHS

Entry Points

1. Website:
Apply.mt.gov: At this site, they can apply for SNAP (Supplemental Nutrition Assistance Program - food assistance), Medicaid and/or TANF (Temporary Assistance for Needy Families - cash assistance). They can check the status of their benefit program, upload documents required for continued benefits, report changes and receive correspondence from our offices. In order to create an account on the online website, they must have an email address.

2. Centralized phone system
1-888-706-1535
This phone system is handled by eligibility staff in our 19 offices across the state. Individuals calling in can either wait on hold to speak to someone or schedule a call back. If scheduling a call back, they have the ability to choose a day/time and our staff will call them back at that scheduled day/time. This method of reaching our staff has the highest volume so most people request a scheduled call back. Individuals calling in can complete an application (with a telephonic signature), report changes, inquire about benefits, etc.

3. Centralized fax system
1-877-418-4533
The centralized fax system is located in Helena. Once the fax is received, it is scanned into our electronic file system and received by our eligibility staff. Applications, changes, information sharing, etc. can be received through this method.

4. Centralized mail system
PO Box 202925, Helena MT 59620
The centralized mail system works very similarly to our centralized fax system. Once the documents are received in the mail, they are scanned into our electronic file system and received by our eligibility staff in order to process for benefits.

5. In person at local office
the link above provides a list of all of our offices of public assistance with their addresses. These offices are staffed Monday – Friday. Also listed on the link are the local offices email addresses – this is yet another way for individuals to reach our staff. They can submit documents in person or via email as well as correspond with our edibility staff. We also have a handful of sites that we provide regular outreach services to – Dillon, Livingston, Choteau, Superior, Pablo, Cut Bank, Fort Benton – most of these sites are visited on a bi-weekly basis.
